Pension Payment Verification Form

You may have recently received in the mail a Pension Payment Verification form with a request for a recent copy of your bank statement. Please note, due to the COVID-19 pandemic, it is NOT necessary to submit this form for the 2020 year.  You will receive a formal retraction letter by mail.
