Great news! We're excited to let you know that our brand-new patient portal, My Funds Portal, is now up and running, starting from August 22nd, 2023. We've made things even simpler for you: your old patient portal, HappyHealthyMe, will automatically take you to our new website, MyFundsPortal.com.

We've been listening to your feedback about the challenges you faced while using the previous portal, and we're thrilled to announce this major upgrade. Our fresh logo proudly represents our four Health Centers and is designed to catch your eye easily on both app stores.

The best part is, you don't need to worry about your login details. Your existing username and password from HappyHealthyMe will seamlessly work on My Funds Portal. Your only task is to grab our new apps, and you're all set!

You will now be able to schedule your own doctor's appointment at a time that is most convenient for you without having to call in!

- Receive information regarding your offsite referrals.
- Access information from your medical records.
- View lab results with your provider's comments.
- Access to your Provider's notes.
- Request new appointments or cancel/reschedule right from your portal account.
- Request medication refills without the need to schedule an appointment with your doctor*.
