Getting Married
Congratulations! To add your spouse to your health coverage, visit the Central Administration Office at 305 W. 44th Street, New York, NY 10036 with the original marriage certificate and your spouse’s original social security card. You and your spouse need to read the benefits package you receive, and become familiar with how to use this health plan. Remember where to go for care and use our 24/7 Nursing Hotline if you are not sure, and our locations are closed: Call 877-TALK-2-RN (825-5276).
Having a Baby / Adopting a Child
1. Notify the Health Center within the first few days after birth, and fax and provide a copy of the hospital discharge papers to the Health Center. This will only secure benefits for your child for their first 30-days. Coverage will discontinue if the second step below is not completed.
2. Notify the Fund Office within the first 30-days of birth/adoption by bringing the child’s original English language translation birth certificate or adoption papers to the Central Administration Office (305 W. 44th Street, New York, NY 10036).
Changing My Address
Your address can be changed:
1. Online through your Patient Portal Account (www.HappyHealthyMe.org) or
2. By filling out a demographics update sheet at any of our Health Centers or
3. In person at the Central Administration Office (305 W. 44th Street, New York, NY 10036 – 4th Floor)
One of the following original documents will be requested for in person changes, and for those moving between boroughs or states. Original proof of residence includes a valid original driver’s license, state-issued ID card with photo, utility bill*, and lease or mortgage statement.
*Gas, electric, cable/satellite TV, internet service, or landline phone bills are acceptable as utility bills; however, cell phone bills cannot be accepted.
Lost My Job
After you lose your job, your benefits will end 30 days after your last day of work. The Central Administration Office (305 W. 44th Street, New York, NY 10036) will also mail you information, rates and a sign up form to continue coverage through a COBRA plan.
Divorce & Separation
Immediately notify the Fund Office to remove your spouse from your coverage by bringing in official divorce papers to the Central Administration Office (305 W. 44th Street, New York, NY 10036).
Severe Injury While Working / Disability
- If you get hurt on the job, notify your employer immediately. You may be eligible for Workers’ Compensation under New York State Law.
- If you become disabled and can no longer work, you may become eligible for Social Security disability benefits and you may want to apply for a disability pension through the Pension Fund if you’re eligible.

Death of Dependent or Spouse

- As soon as you are able to, come into the Central Administration Office (305 W. 44th Street, New York, NY 10036) to update your health plan and beneficiaries if necessary.
Death of a Member
- If you are the family of a member that has passed away, come into the Central Administration Office (305 W. 44th Street, New York, NY 10036) to report the death and find out how to submit claims for benefits payable, if you are the beneficiary and there is a policy in place.

Retirement
If you are now ready to stop working, please come in at least three months before your last day of work to discuss retirement with our Pension Department and Members’ Experience Staff, in the Central Administration Office (305 W. 44th, New York, NY 10036).
